PCBCW Application

The Oregon Academy of Family Physicians seeks to support better collaboration among primary care and community-based organizations (CBOs) through a pilot program partnering with communities to address a community prioritized challenge. This program is open to ALL OAFP members, including medical students, residents, and retired  physicians. Participants will establish enduring working relationships with CBOs working on social drivers of health through mentored development of projects aimed at improving community health needs related to inequities and social drivers of health. Deadline of July 15th.
