AAFP Commission Appointment Information
In July the AAFP will announce the Commission openings for 2009.:
The deadline for nominations will be in October, 2008. Nomination packets need to include a completed fact sheet, photograph, and a letter of recommendation from the OAFP. You will also need to complete an on-line Conflict of Interest form.
If you are interested in one of the Commission positions, please contact Kerry Gonzales, kg@oafp.org, before September 22. We will be submitting all nominations by October 6th.
To fill out the Conflict of Interest form:
- Go to www.aafp.org/conflict.
- Key in AAFP ID numbers and last name or password.
- Press submit
- Select Option 2.
- If there is anything to disclose, type the conflict in the box entitled "Disclosure of Actual or Potential Conflict of Interest" If there are no conflicts, type "none"
- When the "I Agree" button is pressed, the Conflict of Interest form is submitted.
