OREGON'S PATIENT-CENTERED PRIMARY CARE HOME (PCPCH)
Apply NOW to become a PCPCH.
The Oregon Legislature enacted House Bill 2009, which created the Oregon Health Authority (OHA) and established a PCPCH Program. The program in administered by the Office for Oregon Health Policy and Research (OHPR).
The goals of the program are to develop strategies to identify and measure Patient-Centered Primary Care Homes, promote their development, and encourage populations covered by the Oregon Health Authority (OHA) to receive care in this new model.
Practices can now submit an application to be recognized by the state of Oregon as a Patient-Centered Primary Care Home.

HEALTH REFORM- WHAT DOES IT MEAN FOR YOU AND YOUR PATIENTS?

Elizabeth Steiner Hayward, Immediate PastPresident of the OAFP, has been appointed to the Oregon State Senate.
Jill Ginsberg, MD has won the 2012 Glayds McCoy Award for lifetime volunteer achievement.
Dr. Ginsberg co-founded the North by Northeast Community Health Center in Portland in 2006, which has provided free health services to thousands of uninsured adults with chronic health conditions.
“Dr. Ginsberg is an ideal recipient of the McCoy Award given her extraordinary advocacy, self-sacrifice, and work to improve the lives of Multnomah County residents with chronic health problems who face the overwhelming challenge of maintaining their health without access to insurance or significant financial resources,” said Steven Joiner, Chair of the County’s Citizen Involvement Committee (CIC).
